Someone who is both the manager and the promotor initially.... but it depends on the stage of the boxer's career, and the relationships between each of the 4 above. There's not really a straight answer...

Exactly, the top boxers in the world know they can generate the money so they don't need anyone. In recent times Hamed and Hatton have done it and been very successful financially, Calzaghe of course is doing it now as well.
Until then it is the big promoters who have all the clout, but the top fighters can outgrow them.
Ultimately it is the TV companies who have the real clout as they hold the purse strings, but they don't always wield it effectively.
