Apparently it was coined when he hit a sparring partner so hard that the guy said it looked like Lewis had six heads.

Lewis would later be re-christened "Turtle Head" as a result of having to abort a fight mid-contest (and losing by TKO) due to nearly shitting himself and having to race to the toilet.

Much better in that regard is Panomroonglek Kaiyanghadaogym - or, in Thai, Panomroonglek Hadao Grilled Chicken Gym.
