I have some possible corrections for NABF and USBA titles.
FLYWEIGHT
1975-12-10 Willie Jensen (213) def Tony Rocha (299).
You have this match as a USBA title bout. As you know, USBA didn't exist until 1979. I found out that this was an "unofficial" title recognized by The Ring (although USBA might have recognized Jensen's 1975 reign retrospectively).

LIGHTWEIGHT
1985-06-05 Tyrone Crawley (3115) def Charlie Brown (8890)
It was for the vacant USBA lightweight title. You have this as an NABF title bout, but I haven't been able to find any NABF mention regarding this match.

SUPER LIGHTWEIGHT / JUNIOR WELTERWEIGHT
1975-08-29 Adolph/Adolfo Viruet (328) def Monroe Brooks (370)
This one is confusing. Viruet was billed as American or North American champion depending on when & where.
Pre-match advertisement on Las Vegas Review-Journal says "American Junior Welterweight Title" (1975-08-29 pg33), but the post match article says Viruet beat Brooks "to win the North American junior welterweight championship" in the bout "sanctioned by the North American Boxing Federation." (1975-08-30 pg7)
Many other newspapers on the day after the match via Associated Press, including the Miami Herald (pg 227), have "North American Boxing Federation's American junior welterweight title."
Either way, this was not a USBA title unless it was recognized as such retrospectively.
1982-11-17 Bruce Curry (408) def Ronnie Shields (2782) for the vacant NABF title. You have this as a bout for the vacant USBA title, but I haven't been able to find USBA mentioned for this match.
I believe Johnny Bumphus's (18262) USBA title reign was not interrupted during this time.

1995-01-31 Reggie Green (6505) def Jaime Balboa (2092) for Balboa's NABF title. You have this as a USBA title bout, but I haven't been able to find USBA mentioned for this match. Balboa had won the vacant NABF title on 1994-12-13.
I believe Darryl Tyson's (5222) USBA title reign was not interrupted during this time.

SUPER WELTERWEIGHT / JUNIOR MIDDLEWEIGHT
2003-06-19 Tybius Flowers (28787) def Jose Medina (98445) for USBA Atlantic Coast Regional Title, which Flowers held until he died in 2004-03. Not sure if he was recognized as the national champion at any point.

MIDDLEWEIGHT
1977-06-21 Ralph Palladin (18054) def Gene Wells (18075) for the vacant NABF junior middleweight title, not the middleweight title as you currently have.
