I didn't say that was the first incidence, I just said it was my first thought, i.e. the first one that sprang to mind. This is because I happened to watch that when it happened, whereas Johnson-Ketchel was only slightly before my time. Doesn't mean I didn't know about it, FFS. I mean, if somebody said "name a sci-fi movie", I'd more likely say Star Wars than Metropolis, not because I "don't know my history", just because that springs to mind more readily.
Anyway, the OP asked for something specific, and I gave a good example. The fact it was for a "WBO trinket" and ended in a serious injury doesn't detract from the criteria one bit.
